Who Is an Ideal Fit for NCC Assisted Living?

We partner closely with hospitals, social workers, and rehab centers to support seniors who:

  • Are medically stable but unsafe to return home alone
  • Need hands-on support with 2+ ADLs (bathing, dressing, toileting, transfers)
  • Require medication management or cueing for adherence
  • Have cognitive decline, early dementia, or poor safety awareness
  • Have limited family/caregiver support or caregiver burnout

Not appropriate for: residents requiring continuous IV therapy, ventilator support, or intensive behavioral management requiring locked memory care.

Care Tiers & Services

Flexible service tiers allow us to scale support around your patient’s evolving needs—from light assistance to high-touch daily care.

  • Standard Assisted Living: ideal for seniors with moderate ADL needs and stable chronic conditions.
  • Enhanced Assisted Living: for frequent hands-on assistance, high fall risk, or cognitive impairment.
  • Respite & Step-Down Stays: short-term stays following hospitalization or rehab.
  • End-of-Life Support: in collaboration with your preferred hospice agency.

Common Questions from Hospitals, Social Workers, and Rehab Teams

If you don’t see your question here, include it in your referral notes or call our referral line.

How quickly can you accept and admit a new resident?

In many cases we can provide a preliminary acceptance decision within 24 business hours of receiving a complete referral packet, and schedule move-in within 3–7 days depending on family readiness and payer arrangements.

What payers do you accept?

We primarily support private pay and long-term care insurance. Depending on your state, we may accept Medicaid waiver or other assisted living benefit programs—contact us for current payer specifics.

Can NCC Assisted Living take residents discharging with home health or hospice?

Yes. We frequently coordinate with home health and hospice agencies for residents who need higher levels of clinical oversight while benefiting from a residential environment.

What information do you need with each referral?

At minimum, we request a recent H&P, current medication list, nursing notes with ADL needs, any PT/OT notes related to mobility and fall risk, behavioral notes if applicable, and contact information for the responsible party or guardian.

Can we tour or audit your community as a partner?

Absolutely. We welcome partner site visits, safety reviews, and joint case conferences so you can confidently include us in your discharge planning toolkit.
